In der Datenbank der bei der Entwicklung des Services verwendeten öffentlichen Handbücher wurde eine durch Studienergebnisse statistisch belegte Wechselwirkung nachgewiesen, welche entweder zu den negativen Wirkungen auf die Gesundheit des Patienten führen oder das gemeinsame positive Effekt verstärken kann. Es ist notwendig, einen Arzt zu befragen, um mehrere Arzneimittel gleichzeitig einnehmen zu können.

Konsument:

Terfenadin ist nicht mehr auf dem Markt in den USA. Nehmen Sie nicht Terfenadin mit anderen Medikamenten.

Fachmann:

KONTRAINDIZIERT: Coadministration mit Azol-Antimykotika kann erheblich erhöhen die Plasmakonzentrationen von Terfenadin. Der Mechanismus ist Azol-Hemmung von CYP450 3A4, das Isoenzym verantwortlich für die metabolische clearance von Terfenadin. Hohe Plasmaspiegel von Terfenadin wurden im Zusammenhang mit der Verlängerung des QT-Intervalls auf das EKG; ventrikulären Arrhythmien einschließlich ventrikulärer Tachykardie und torsade de pointes; Herzstillstand; und den plötzlichen Tod. Innerhalb der Azol-Klasse, Ketoconazol und Itraconazol sind als sehr potente Inhibitoren, während fluconazole ist vergleichsweise schwach und in der Regel verursacht klinisch signifikante Wechselwirkungen mit CYP450 3A4-Substraten nur bei Dosierungen von 200 mg/Tag oder mehr. In sechs gesunden Freiwilligen, Fluconazol (200 mg täglich für 7 Tage) hatte keinen Einfluss auf die steady-state-plasma-Konzentrationen von Terfenadin (60 mg zweimal täglich für 15 Tage) oder der kardialen ¤ re Repolarisation gemessen als Holter-die QTc-Intervalle. Jedoch eine andere Studie mit Fluconazol-Dosierungen von 400 und 800 mg/Tag zeigte signifikante Erhöhungen der Plasmaspiegel von Terfenadin.

MANAGEMENT: Die Verwendung von Terfenadin mit den meisten Azol-Antimykotika, einschließlich Fluconazol, auf mehrere Dosen von 400 mg oder höher ist, gilt als kontraindiziert. Loratadine, cetirizine, oder Fexofenadin kann sicherer alternativen während der Therapie mit Azol-Antimykotika.
