Es ist notwendig, einen Arzt zu befragen, um mehrere Arzneimittel gleichzeitig einnehmen zu können.

Konsument:

Es ist möglich, dass brivaracetam beeinträchtigen die Wirkung von clopidogrel bei einigen Patienten. Dies kann das Medikament weniger wirksam bei der Verhinderung von Herzinfarkten und Schlaganfällen. Sprechen Sie mit Ihrem Arzt, wenn Sie irgendwelche Fragen oder Bedenken haben. MONITOR: Coadministration mit Inhibitoren der CYP450-2C19 verringern möglicherweise die Wirksamkeit von clopidogrel, dessen gerinnungshemmende Wirkung hängt zum Teil auf bioaktivierung durch das Isoenzym zu einem pharmakologisch aktiven Metaboliten. Dies steht im Einklang mit Studien, die berichtet, verminderte Wirksamkeit von clopidogrel und schlechterem klinischen Ergebnis bei Patienten, die häufige genetische Polymorphismen in CYP450 2C19, resultierend in reduzierter oder fehlender Enzymaktivität. Die Interaktion wurde untersucht mit Omeprazol, einem potenten CYP450-2C19-inhibitor. In 72 gesunden Probanden verabreicht clopidogrel (300 mg initialdosis gefolgt von 75 mg/Tag) allein und mit Omeprazol (80 mg) gleichzeitig für 5 Tage, systemische Exposition der aktiven Metaboliten von clopidogrel sanken um 46% (Tag 1) und 42% (Tag 5) bei coadministration mit Omeprazol, während die mittlere Hemmung der Thrombozytenaggregation (IPA) vermindert sich um 47% (24 Stunden) und 30% (Tag 5). Ähnliche Ergebnisse wurden gemeldet, wenn die gleichen Dosen von clopidogrel und Omeprazol verabreicht wurden 12 Stunden auseinander in einer anderen Studie. Daten nicht verfügbar sind für weniger potenten Inhibitoren von CYP450-2C19. Jedoch ineffektive Hemmung der Thrombozyten-aggregation wurde berichtet im Zusammenhang mit einer möglichen Interaktion mit Amiodaron, dessen aktiver Metabolit, desethylamiodaron, hat sich gezeigt, zur Hemmung von CYP450-2C19 in vitro.

MANAGEMENT: Basierend auf den vorhandenen Daten, kann es ratsam sein, genau zu überwachen, die therapeutische Wirksamkeit von clopidogrel bei gleichzeitiger Behandlung mit CYP450-2C19-Hemmern.
