In der Datenbank der bei der Entwicklung des Services verwendeten öffentlichen Handbücher wurde eine durch Studienergebnisse statistisch belegte Wechselwirkung nachgewiesen, welche entweder zu den negativen Wirkungen auf die Gesundheit des Patienten führen oder das gemeinsame positive Effekt verstärken kann. Es ist notwendig, einen Arzt zu befragen, um mehrere Arzneimittel gleichzeitig einnehmen zu können.

Konsument:

Astemizol und Terfenadin sind nicht mehr auf dem Markt in den USA. Nehmen Sie nicht astemizol oder Terfenadin mit anderen Medikamenten.

Fachmann:

KONTRAINDIZIERT: Coadministration mit protease-Inhibitoren (PIs), besonders ritonavir, kann erheblich erhöhen die Plasmakonzentrationen von astemizol und Terfenadin. Der Mechanismus ist PI Hemmung von CYP450 3A4, das Isoenzym verantwortlich für die metabolische clearance von beiden astemizol und Terfenadin. Obwohl die Interaktion wurde nicht speziell untersucht, mit den verfügbaren PIs, hohen Plasmakonzentrationen von astemizol und Terfenadin (z.B. aufgrund von überdosierung oder Wechselwirkungen mit anderen potenten 3A4-Inhibitoren wie Makrolid-Antibiotika und Azol-Antimykotika) wurden im Zusammenhang mit der Verlängerung des QT-Intervalls auf das EKG; ventrikulären Arrhythmien, einschließlich ventrikulärer Tachykardie, Kammerflimmern und torsade de pointes; Herzstillstand; und den plötzlichen Tod.

MANAGEMENT: Angesichts des Potenzials für schwerwiegende und lebensbedrohliche unerwünschte kardiale Ereignisse im Zusammenhang mit erhöhten Plasmakonzentrationen von astemizol und Terfenadin, die Verwendung dieser Wirkstoffe bei Patienten, die mit protease-Hemmern gilt als kontraindiziert. Loratadine, cetirizine, oder Fexofenadin kann sicherer alternativen während der Therapie mit PIs.
